How it's calculated
total dose = weight (kg) × dose per kg
volume = total dose ÷ concentration
Pounds are converted to kilograms first (1 lb = 0.453592 kg). The volume step uses the classic “desired over have” idea: divide the dose you need by the concentration you have on hand. This is a quick reference only — independently verify every dose against the order, a drug reference, and your facility's policy before administering.
